Stevenson Memorial Hospital (SMH) is asking residents of New Tecumseth and surrounding communities to nominate an SMH nurse – an RN, RPN or Nurse Practitioner – for the SMH Award of Excellence in Nursing.
If you have been positively impacted by the dedicated care of an SMH nurse, SMH would like to hear from you!
Nominators are to provide a personal account explaining why this nurse deserves to be recognized with the Award of Excellence in Nursing. This may include a brief description of their qualities, care of patient, specific examples of the nurse’s efforts and dedication, and the difference they have made to your life or the lives of others. (NOTE: Your story must have taken place between April 1, 2023 and March 31, 2024. Nurses may not be nominated by a member of their family).
Please ensure the following information is included:
• Name of the nurse
• Where the care or association with the nurse took place (specify hospital department(s)
• Dates of care provided, or date of association with the nurse (if possible)
• Your name and daytime telephone number
• Your relationship to the nurse (i.e. patient, family member of patient, friend of patient, former student, or colleague)

A panel of judges will select a recipient, who will be announced and presented with the award during Nursing Week (May 5-11).
